fix up some confusion with filesources' _origin and _file_is_new members. if _origin is set, it means that the file is "external" to the session (aka "embedded") and for some purposes this is more significant than _file_is_new. rename SourceFactory::createReadable() to ::createExternal() to more clearly indicate its purpose; remove never-supplied "origin" argument from SourceFactor::createWritable(). Fixes problems caused by 864ce8f0

major fixes for MIDI patch change and note undo/redo. Patch change handling was completely broken because of the use of absolute floating point comparisons for time comparison, and serialization/deserialization of patch change property changes was borked because of int/char conversions by stringstream. Note undo/redo would fail for note removal if a note had been moved and/or had its note number changed as the next operation after it was added, because time-based lookup would fail. Similar small changes made for sysex messages, which just needed the musical_time comparisons and nothing else

initial working version after changes to MIDI channel filtering. may affect metering input too. testing not yet finished
this commit merges many deep changes in ardour's internal architecture,
combined with a total redesign of how MIDI channel filtering works.
data in a track used to flow from JACK port buffers to diskstream's ringbuffers
and was then copied from the ringbuffers into a BufferSet for use during
Route::process_output_buffers(). The butler thread would handle the movement of
data between the ringbuffers and disk.
with this commit, data now flows from JACK port buffers into the BufferSet used
for Route processing, and is copied from the BufferSet into the diskstream's
ringbuffers (the butler thread continues to handle interactions with disk as
usual).
this change allowed a dramatic consolidation of code and simplification of most
aspects of Track/Route::roll() and Track/Route::no_roll(). in particular, see
Route::fill_buffers_with_input() which now concisely describes how we move data
from JACK port buffers into the BufferSet for all Route types (including Tracks).
this work was initially motivated by changing MIDI channel filtering so that we
can process capture and playback independently. there is now a very clean
pathway for this - see MidiTrack::roll() (NOTE: This needs implementing in the
no-roll case too - a TODO item).
the channel selector for MIDI tracks has been moved out of the track header and
is now accessible via the context menu. more work is likely here, to make it
(more) obvious to the user when filtering is going on.

initial, prototype modifications to permit compilation of local libraries as static libs. required a "fix" to libs/pbd/debug.cc to even get the program up and running, and still does not work due to issues with boost::shared_ptr::enable_shared_from_this. controlled by configure-time --internal-{static,shared}-libs, set to shared by default (as has been the case for years)

Handle mutiple export files with the same extension but different format.

If multiple filenames have the same extension, append the format name
to the filename. This still requires a bit of extra logic to be optimal,
as the format name will now be added in some situations where it is not
needed. However, this is better than producing a broken file...

In rev 12740 FadeIn/Out became stateful properties which are automatically
saved when Region:state() calls Stateful::add_properties().
AudioRegion::state() called Region:state() AND Stateful::add_properties()
which added a 2nd redundant copy to the XML.
Finally AudioRegion::state() adds custom serialization for Fades.
Furthermore this custom serialization also used names which did not
match the Property name.

Remove debug print about transport sync.

more i18n fixes, this time removing a hack that broke i18n in bundles, and also only initializing NLS stuff if ENABLE_NLS is in effect. note that if translations are disabled by the user, we set the localedir used to look for message catalogs to a dir that is assumed to not exist (/this/cannot/exist) which should block all translation from happening.

Fix MIDI loop recording.

This changes how things work a bit, but I am committing it for 3.0 since the
previous revision often crashed (and never worked), this one seems to work
fine, and the code is quite a bit more cogent.
I have tested the following use cases with live input and audible output:
* Non-loop recording, armed before roll
* Non-loop recording, arm while rolling
* Loop recording, armed before roll
* Loop recording, arm during roll
In the last case, the source/region is created starting at the loop start
rather than the current transport frame as usual so time makes sense when it
wraps around.
See the documentation added to the code for details, but the basic idea here is
to simply push MIDI events to the source with increasing monotonic time,
ignoring looping altogether. Essentially we pretend the loop does not exist,
but the source knows all the details so it can implement whatever behaviour is
appropriate.
Currently, this is simply recording a complete non-destructive copy of the
input, which is a good thing. Perhaps not what the user expects of loop
recording, but at least it works and is one sensible option. We will need to
add more later.
Display while recording is a little bit wacky, but whatever.

Actually flush to disk continuously while MIDI loop recording.

Previously, this calculation was based on the current transport frame and last
time, both absolute, with the effect that loop recording would flush the first
time and never again.
The fix is to keep track of the actual duration of the capture, ignoring the
transport position, and flush as that reaches the threshold.

The Ardour test suite does not pass. I commented out old crossfade stuff, but
I am not familiar enough with the parts that fail to fix it. It might be a
good idea for someone to look into this. Ideally we'd have the test integrated
into everyone's workflow, but they add quite a few files to compile...

Do this via a simple MasterDeviceNames::note_name() function. The same really
needs to be done for program names, this stuff is absolutely brutal to use.

Add convenient method to MIDINameDocument for getting the names for a device.
Tolerate comments in MIDINameDocument ControlNameList.
Can't remove the MIDI name code just yet, since it's still erroneously used by
Automatable::describe_parameter(). This is the cause of a bug where the name
on the automation lane does not match that in the menu.
The plan is to make a very simple pure abstract interface for getting MIDI
names, and make it possible to set one for Automatable (or perhaps pass it to
describe_parameter()). Thus we'll be on the way to supporting names from
sources other than midnam files, namely plugins.

Since this is usually not the case, showing this all the time as before was so
confusing everyone thought it was broken (myself included).
Changing that show_all() to a show() might have consequences, but it seems to
work fine and we really shouldn't be using show_all() anyway.

remove timecode subframe configuration
...
There is no actual use-case to set it to anything else than 100.
timecode subframes are only used for ruler marks and the snap to grid.
libtimecode is still /aware/ of the subframe setting.

remove use of a boost::fast_pool_allocator without a mutex for Evoral::ControlList. this was not thread-safe and evidence from googling around suggests that it is actually not faster for this sort of use than std::alloc. removing this fixes bizarre errors caused by multithreaded use of the singleton pool that backs the allocator, which started to become more clear during experiments on 128 track sessions

crossfade hack and slash. removed overlap checks, overlap mode, default length, fade_is_xfade, fade_is_short, and other leftovers from previous crossfade models. Draw crossfade drags in realtime so fade_line is no longer needed. remove code for hiding crossfades during a drag. moved crossfade rect to top layer so crossfade lines dont grab mouse focus. drag-trim-with-fixed-fade-anchor is partially implemented and needs discussion

sometimes, you just have to do what a windows programmer would do. add a global signal, ARDOUR::GUIIdle() which can be used in the middle of long-running backend operations to give the GUI a chance to update. use it while adding routes.

The data from the lower layer(s) was not faded out because
the reversed gain curve was incorrect because ControlList:add()
inserts anchor points.
a call to reverse_curve() for a linear fade produced:
INPUT: [when,val] {[0.0, 0.0], [300.0, 1.0]}
OUTPUT: (reversed) {[0.0, 0.0], [0.0, 1.0], [1.0, 0,0], [300.0, 0.0]}
solution: use fast_simple_add() instead.

Deliver MIDI events to atom ports that support it, merged with transport events.

Eliminate delivery of non-MIDI events to Ardour MIDI buffers.
Fix the general mess of LV2 port flags and types.
Unify old MIDI API and new atom API MIDI stuff.
Making this work required changing the way we do LV2 MIDI slightly: instead of
BufferSet::get_lv2_midi doing the translation automatically, it returns an
empty buffer and the caller is responsible for actually filling it. This
allows the LV2 code to iterate over the MIDI and transport changes in one go to
merge them into the buffer at the correct times.
Synth plugins using the new atom API now work and can get accurate transport
information along with the MIDI that drives them.

This might use a bit more memory than it could, but it works reliably for me
loading a large Ingen patch inside Ardour as an LV2 plugin, which I'm pretty
sure is by far the most high volume such communication out there.

Sent precise transport information to LV2 plugins via events.

We send the full transport state (frame position, BBT time, transport speed,
meter) to the plugin:
* At the start of a cycle whenever a relocate or transport speed change has
occurred
* On every occurrence of a meter change within a cycle
This means the plugin gets a sample accurate meter/tempo map, even if the meter
changes in the middle of a cycle. However, this is not quite right yet: things
can get wonky if the tempo map is edited while rolling, since this code will
not detect the change and fail to update the plugin at the start of the cycle.
Other changes:
* Factor out TempoMetric::set_metric() and simplify some tempo functions
* Clean up LV2 URID stuff

I have not proved if the iterative process in
CubicInterpolation::interpolate() is identical to
(nframes * fabs(_actual_speed)), however ceil()
of it is empirically always larger.
Also, the cubic interpolation needs +2 samples.

The reason for this is that the LTC generator uses the slave's
resolution as watermark for max-drift, resync. A possible drawback:
session-process will force silent rolls if the delta is larger
than the resolution. LTC has a carrier freq of ~2kHz, choosing
half of that seems a good compromise.

fix issue 0005120: dont treat percussive mode differently than sustained mode except in GUI. Leaving out all note offs crashes LinuxSampler and totally drowns my Yamaha PSR-S900 by stealing all its available voices. Code is #ifdef-ed out until a more thorough discussion would conclude the removal of percussive mode from the MIDI model, which IMHO makes sense. I don't see any virtue in leaving out note offs since it only creates problems.

* replace 1st order PI-controller with 2nd order DLL
The 2nd DLL is needed because the jack-process callback effectively
quantizes the transport-speed to integer audio-frames (speed * nframes).
This leaves a delta on every cycle because the remainder from previous
cycles is not compensated for.
Theoretically it is possible to merge the two DLLs into one.
However, with callbacks coming from by two independent threads
it is cleaner to track the MTC and JACK engine-alignment speeds
independently.

LV2: amend previous commit, respect replicated plugin instances

When a plugin is replicated (eg. mono plugin on a stereo-track),
configure_io() is only called for the first instance.
The 2nd instance will not have dedicated buffers but re-use copies
of the first instance. Should the 2nd instance use a LV2 worker-thread or
local communication ports they can not be shared with the first instance.
-> this patch allocates Atom-buffers for each instance.
PS. this still needs more work; there is only one UI instance for
all plugin instances. messages from the UI should go to all
instances.

Report correct time instead of old time when locating as Jack transport master.
...
I encountered this today writing a transport slave, but it seems to be the same
problem as issue #743 from 8 years ago. The issue is easier to see with a
transport slave client that prints any transport change whatsoever, for example
if the current location is some point well into the session and rewind to start
is pressed, Ardour reports the old position, not zero. With this change, it
reports zero as expected.
See comment about why this was happening. If locating here is evil for some
reason, then some other way of making jack_timebase_callback report the target
position is required. Contrary to what the old comment below this change
suggests, follow_slave() does not update _transport_frame in time.

Fix Jack transport master after session load.
...
Transport master was not being taken because when the configuration variable is
set, AudioEngine::set_session() has not been called yet so
AudioEngine::reset_timebase() did nothing (the transport stuff at the beginning
of Session::when_engine_running() is questionable for the same reasons).
I am not sure if this is correct, but I have failed to more thoroughly figure
out the issue. At least Ardour actually sets Jack transport information now.
However, there are some discrepancies between Ardour time and Jack time, as
reported by Session::jack_timebase_callback().
Perhaps related, the transport master menu item does not restore correctly.
Previous to this commit, the only way to make a loaded session take transport
master was to "set" this out of sync item (which did nothing), then unset it,
then set it again, which would finally actually cause the configuration change.

drastic rethink of the relationship between remote control ID and route order keys. unless the user explicitly switches to UserOrdered, Route::_remote_control_id is an unallocated pointer, and Route::remote_control_id() simply returns a value based on the relevant order_key() value. Also, change the key used in the Route::order_keys std::map<> from a string to an enum, since there is no evidence that we are benefitting from the theoretical benefit of using a string. Generally tidy up allocation of order keys so that the master and monitor busses always get a "special" MixerSort key value, based on the MMC ID for master (already defined within Ardour), and all other tracks/busses start at zero. Syncing keys between editor and mixer will leave the MixerSort key for the master and monitor bus alone, reflecting the fact that we display these in their own distinct parts of the GUI and they are not orderable like other tracks or busses within the mixer window
