Corrected error in Windows MIDI system description. Removed information regarding configuring JACK, and replaced with a note that JACK is now considered an advanced use case, and linked to jackaudio.org for users who need the routing capabilities of jack. Removed the MIDI on Linux and MIDI on OS X sections from the master document since those primarily discussed JACK.
