b35518e212 switch from boost::{shared,weak}_ptr to std::{shared,weak}_ptr 
This is mostly a simple lexical search+replace but the absence of operator< for
std::weak_ptr<T> leads to some complications, particularly with Evoral::Sequence
and ExportPortChannel.

Ignore saved port-name when loading ctrl-surface state 
In some cases old and/or conflicting port names were saved
with the session (e.g. "Faderport" for FP1,8). Loading old sessions
then merges this state into the config, which could lead to
port-registration failure when the surfaces was enabled.

b5e0b5b09f PortManager::unregister_port() must be called with process lock 
This fixes "Failed to register <surface> port" when re-loading a session.

f8875df18f Several changes to Mackie support. Breaks existing Mackie state on disk but not 
in any way that causes problems, just loss of pre-existing connectivity.

1. retain state of current device (and serialize to disk) when switching
devices, and restore that state when switching back to it.
2. fix port and surfacenaming.
3. fix bundle assembly so that all ports (for multi-surface combos) work.
4. rationalize master position numbering
5. add small sleep before starting device handshake after reconnection. This
is ugly but seems to be necessary, unfortunately.

8f091de64b for MCP devices that don't handshake, turn them on as soon as we call ::connectivity_established() so that _active == true, and we will actually write to them

eaa7cc5a73 fix up some const-ness issues starting from Evoral::Event::set(), and intersect with removing Mackie..MidiByteArray::bytes() method which made a copy of the data every time we wrote it

33140f3267 add support for IP MIDI (multicast MIDI over IP UDP sockets) to ardour and use it if requested inside MCP code. required renaming the pre-existing MIDI::Port as MIDI:JackMIDIPort - MIDI::Port becomes the base type for both JackMIDIPort and IPMIDIPort

a7dc433498 Delete MIDI port objects when the MackieControlProtocol is torn down, so that it can be recreated without attempting to create duplicate JACK port names. Should fix #3886.
