* changed the timecode display char selection for update algorithm as chars are sent one by one and not all right most;
* implemented method of showing timecode at the mackie to better deal with the differences between Ardour's foramts and Mackie's, i.e, use spaces in place of the zeros that had no meaning;
* preventing timecode display updates when the surface isn't yet active.

add a fix for the one problem case from the last commit: region splitting, where we don't announce regions until the entire edit op is done, but for naming purpoes each new region created needs to be in the map. likely that i will commit a better, cleaner fix for this later

Remove over 500 unnecessary includes (including 54 of session.h).

It's slightly possible that this causes trivial build failures on different
configurations, but otherwise shouldn't cause any problems (i.e. no actual
changes other than include/naming/namespace stuff). I deliberately avoided
removing libardour-config.h since this can mysteriously break things, though a
few of those do seem to be unnecessary.
This commit only targets includes of ardour/*.h. There is also a very large
number of unnecessary includes of stuff in gtk2_ardour; tackling that should
also give a big improvement in build time when things are modified.

Use a list of ControlPoints to hold the automation selection,

rather than a time range. This makes more sense now that we
display every point on an automation line, rather than just
a subset. Makes the code a fair bit simpler, and should fix
some unexpected behaviours, especially when cutting automation
points.

Fairly major change to the way in which crossfades are handled;

they are now done with region fades, rather than separate objects.
After this commit, Ardour will try to convert your session files
to the new crossfade format, but will make a backup in your
session folder first.
If you have works in progress using Ardour 3 it is
***STRONGLY RECOMMENDED*** that you back up session files before
updating to this commit.

Anywhere that deletes regions needs to use a rdiff() on the playlist

for the undo history, so that changes to regions' layering_index
get stored in the undo record. Make Playlist::update use add_region_internal
so that undone regions don't have their layering_index corrupted.
Setup layering indices on relayer() so that deletion of regions
causes an update.

somewhat illogical and temporary workaround to the problems with consolidate & bounceable. it turns out that region bouncing never replaces the in-place region, so bounceable() doesn't matter; range bouncing only needs to check bounceable() if we're going to replace material in place and the op is with-processing. reword the error dialog too

Remove extend-range-to-{start,end}-of-region and replace with move-range-{start,end}-to-{previous,next}-region boundary after discussions with Chris. Seems to make more sense, and works without the need for a region selection, which is fiddly to adjust when one is in range mode.

Stop bounce / freeze on tracks that have more outputs than inputs and so cannot record all the outputs in their diskstreams. Fix buffer shortage when bouncing tracks whose processing chains temporarily need more buffers than there are inputs. Fixes #3573 .

Clean up Editor's update of various things by:
1. merging Editor's handlers of SuperRapidScreenUpdate.
2. separating out work to be done on this update and work to be done
on a locate.
Hopefully easier to understand this way.

Clean up range menu a bit. Make a couple of range actions happen to all tracks if no tracks nor regions are selected. Stop involvement of the edit point in those same actions as it doesn't seem to make much sense.
