Paul Davis b35518e212 switch from boost::{shared,weak}_ptr to std::{shared,weak}_ptr 
This is mostly a simple lexical search+replace but the absence of operator< for
std::weak_ptr<T> leads to some complications, particularly with Evoral::Sequence
and ExportPortChannel.

Mads Kiilerich 728e463d01 AudioClock: make last_when public and use it instead of current_time 
Having current_time as a public alias of the private last_when did not
add any clarity to the abstraction of last_time. A small step, but still
room for improvement.

Paul Davis 45e21de209 implement and adopt new conventions for AudioClock use 
The class now has two separate methods for setting a duration or a point
value. They MUST be used appropriately, because their behavior is different.

When ::set_duration() is used in timecode mode, an extent (inclusive-end
length) is shown rather than a length.

Some objects, such as the TimeInfoBox, now deliberately shown an inclusive end
for their "end" clock, but this not universally followed, pending more feedback
from users and investigating of conventions in other DAWs.

Paul Davis 373a73cb51 fix selectability of region-associated automation (i.e. MIDI) 
The fix here is really just dropping the use of _offset when computing the session position
of a control point. This was just an arithmetical error.

However, session_sample_position() was redundant and just caused more work, so this
method was removed, and only ::session_position() is now used.

In addition, several closely related places now use C++11 (or later) "auto"
syntax for iterating over containers, for cleaner looking code

Copyright-holder and year information is extracted from git log.

git history begins in 2005. So (C) from 1998..2005 is lost. Also some
(C) assignment of commits where the committer didn't use --author.

nick_m fb761a6fa7 Rework time info box updates on region selection change 
Should provide better performance as we now only listen for changes in the
selected regions. Testing every changed region to see if its in
the selection was not working very well under some circumstances.

Robin Gareus 2562a5ce3b Save/Restore TimeInfoBox clock modes 
The clocks are not transient, so ARDOUR_UI::store_clock_modes takes
care of saving the state and the clock restores itself.

Carl Hetherington a4434809e1 Use a list of ControlPoints to hold the automation selection, 
rather than a time range.  This makes more sense now that we
display every point on an automation line, rather than just
a subset.  Makes the code a fair bit simpler, and should fix
some unexpected behaviours, especially when cutting automation
points.

Paul Davis 8ad4133d26 don't cache width & height for CairoWidget; provide option for all CairoWidgets to not draw their backgrounds; provide a static method to allow any (Container, probably) widget to provide background color to a CairoWidget without the requirement that it have its own window; remove existing cursor from clocks; clock text no longer has background attribute set, to correct a mistake in my understanding of pango attributes
